Print and complete THREE COPIES of the form from this website. Copy 1 is for YOUR record. Copy 2 is for the Court record. Copy 3 MUST be attached to Sheriff's Entry of Service and returned to the court for service. COMPLETE YOUR PAPERWORK USING A BLACK INK PEN. 

1. Write YOUR full legal name as the Plaintiff.
2. Write the full legal name of the Defendant.
3. Write the full legal name and address of the person to be served.
4. DO NOT WRITE BELOW TITLE "SHERIFF'S ENTRY OF SERVICE." This section will be completed by the process server.

A SHERIFF'S ENTRY OF SERVICE IS REQUIRED FOR ALL ACTIONS IN MAGISTRATE COURT.
